Please fill out this form to request transcripts. Email Christy.Garrison@dcssga.org for assistance. 

*You must fill out a SEPARATE RESPONSE FOR EACH SCHOOL you are requesting transcripts be sent.  Transcripts for Georgia schools can be requested at no cost through www.gafutures.org.  All transcript requests submitted through this form are processed for $3 per school.  You will be given a link to pay after you press submit.
